Postage at carrier cost means the fulfillment operator bills you exactly what the carrier charged for the parcel, with no markup added. It is a pass-through line on your invoice rather than a shipping rate the operator sets.
How to tell whether it is really pass-through
Shipping is the easiest place in a fulfillment bill to hide margin, because you cannot see the carrier's invoice and the number looks like a fact. An operator quoting flat shipping per order is not necessarily overcharging, but they are certainly not passing through, because carrier pricing varies by weight, zone, and service and a flat number cannot track it.
The test is whether the number moves. Two identical vials going to two different states on the same service should not cost the same, and if they do, the rate is being set rather than passed on. The other test is simply asking for the carrier reference on a line you want to verify, which a pass-through operator can produce because the number came from a carrier record.
Pass-through cuts both ways, and honestly so: surcharges pass through too. A residential delivery fee, a fuel surcharge, or an address correction fee appears on your bill because it appeared on the carrier's, which is the same mechanism working in the direction you like less. What your invoice line looks like
Per unit shipped you see three numbers: the wholesale price of the vial, $1.00 to print and apply your label, and postage at what the carrier charged. There is no pick fee and no pack fee added on top, so the arithmetic on a unit is just those three lines.
